The Occupational Performance History Interview II (OPHI-II) is a three-part assessment used in occupational therapy evaluation to explore the client’s life history in the areas of work, play and self-care. The OPHI-II, a revised version of the Occupational Performance History Interview (OPHI), uses a semi-structured interview focusing on three areas of occupational adaptation: occupational identity, occupational competence, and the impact of occupational behavior environment. The semistructured interview addresses five domains needed to rate the areas of occupational adaptation using a 4-point scale. Formal training is not required to administer the OPHI-II, and scoring is based on the therapist clinical judgement and interpretation of interview
