Assessment instruments like the Fulmer SPICES assessment and the FANCAPES model point to respiratory distress as a serious concern in Mrs. X's case. Mrs. X presents with symptoms of compromised ventilation, including tachypnea, mild wheezing, diffuse rales, and speech disturbances. The diagnosis of Mrs. X relies heavily on ventilation because her respiratory distress is a direct result of the body's incapacity to exchange carbon dioxide and oxygen sufficiently. The diagnosis came to a conclusion following more tests such as focusing on respiratory assessment which included the inspection of the level of consciousness, breathing rate, pattern, effort used, accessory muscles being used, cyanosis, chest configuration, and symmetry of chest when it's expanding. When assessing the level of consciousness, the patient should be alert and cooperative.
