In the psychotherapy field, therapist use many different types of theoretical orientations and techniques. Based on the information presented to me, I can see that Bob is untidy and unkempt. The patient appears to believe in his mind, that the police are after him, but he does not reveal why they are after him and he is having difficulty deciding what is reality and what is fiction. It is important to get the medical history in an effort to identify past problems experienced by the patient and to gauge the presenting behaviors being displayed. You are faced with a few issues, preventing a diagnosis, but based on the behavior of the patient, his pressured speech and perception that you are the police, he will not be capable of expressing his true feelings, in fear that you are out to get him. The fact that he stated that he has been in psychiatric treatment for years, suggest that his disorder is severe, because of his extended stay in a psychiatric facility. Since he will not provide the consent for a release of his medical records, it may be possible to use the Psychodynamic Diagnostic Manual, which allows therapist to describe and categorize patterns of personality, personal experiences and unique profiles of the psych.
