Speech-Language Pathology Evaluation

Session Type: Evaluation Length of session: 12 minutes SUBJECTIVE.

History: Mr. XX is a 32-year-old male with a known diagnosis of Flaccid Dysarthria. XX was working in the park approximately three months ago when a severe storm hit the area. While trying to clear fallen trees and debris, he was struck by a large tree limb, causing him to fall and hit his head on the ground; this crushed his skull, causing nerves in the base of his skull, multiple skull fractures, two middle ear bones were broken, along with his cheekbones and sinuses. Otherwise, the speech was primarily normal.

Oral motor exam: Face symmetric. Lip and rounding weakness and difficulty moving lips for speech are apparent. The jaw opened at the midline. The tongue protruded at midline, and lateral lingual movement was expected in the range of speed. Soft palate elevation was symmetrical and mobile on phonation and gag reflex was normal. Difficulty with smiling, rounding lips to whistle, and puffing of cheeks. Frequent protruding of the tongue due to dryness of the lips. No challenges in chewing and swallowing, occasional spillage of liquids.

Assessment

Impressions: Mr. XX presented with severe flaccid dysarthria. I discussed strategies such as strength training for the articulators, decreasing background noise while engaging with his peers, slowing down their speech, having face-to-face conversations, and over-articulation of sounds to produce clarity in words and sentences, along with using gestures, writing or using the computer to compensate when needed.
