It is good to keep the record detail date and time specific incident and the issue. The well-being and the safety of individuals is very important and they should be in a safe environment.
If the complaint is about a colleague it should be reported to the team leader, if the issue or concern is not properly adequately addressed it should result in whistleblowing.
