She states she has had a history of sensitivity to certain antibiotics that cause emesis, however she was unaware of what antibiotics those were. She is currently oriented to person, place, and time. She is alert and able to communicate any complaints about the symptoms she is experiencing. Her lungs are clear in all locations posterior and anterior. She is currently wearing SCD to help with the prevention of thrombosis. She currently has normal saline running at 150ml/hour and has a 22guage intravenous line in her right arm. Her only current complaint is pain in her right shoulder which she rated as a 4/10. Patient stated that this is not a new onset, but something she always deals with. My goal for the patient is, patient will verbalize risk factors for urinary tract infections and measures to take to prevent them. Nursing interventions used to assist with meeting this goal include assessing any current knowledge the patient has about urinary tract infections. After assessing her current knowledge I will be able to develop a teaching plan that includes, educating in the importance of attempting to void often to prevent urine from becoming stationary in the urinary tract. I will also educate on the importance of good personal hygiene including the correct method of wiping after voiding and having bowel movements. It is important to wipe from front to back due to how close the anus and vagina are to the urinary meatus. It is important to educate females on this to prevent the spread of any bacteria from the anus to the vagina and urinary meatus.
