DAILY ACTIVITIES

When the claimant wakes up, she feeds and cares for her dog. She gets her son ready for school and makes sure he gets on the bus. She has some neighbors that come over two or three days out of the week to have coffee. She reports that for every good day she has two to three bad days. She cares for her son when he arrives home from school. In addition to socializing with neighbors over coffee, she talks to her best friend on the telephone everyday. The claimant and her husband are responsible for the household cleaning, cooking and shopping. She is able to do these tasks depending on the state of her mental health at the time.

The claimant had difficulty concentrating today. She she reports struggling to concentrate at work. She is easily distracted, has difficulty multitasking, in part related to her bipolar disorder and begins, but cannot finish tasks due to getting sidetracked. The claimant loses her attention when she tries to focus on tasks or responsibilities that need to be accomplished. The claimant is impaired in concentration.

When she would have a bad day at work, she would become upset and angry. She would try to take a break and go to the bathroom to cry. After work, she would go home and “crash” or go to sleep. Before her sobriety, she would use and abuse substances after a bad day at work. She attended counseling sessions, while she was growing up. She went to sessions for several years. She had a social worker that she was working with for the past eight years. She recently fired him. She was admitted into Riverside Hospital in 2005 for psychiatric care. She was also admitted into a hospital at OSU for psychiatric care in 2010. Each of these admissions lasted three to four days. She is currently prescribed Citalopram, Oxcarbazepine, and Alprazolam. The claimant is impaired in stress
