To improve patient outcomes and reduce adverse events, it is critical to detect quality issues in the healthcare system. One quality issue is when patients delay seeking treatment for worsening heart failure, which can lead to serious consequences such as increased mortality and heart failure-related adverse events. In this essay, I will address the following questions: What constitutes a quality issue in healthcare? How can one determine if there is a quality issue? What is the objective of improvement? Achievable: Educate people about the need to seek care early for signs and symptoms of heart failure and provide resources to help people get care as soon as possible. Relevant: Improving timely care for heart failure symptoms is crucial to prevent complications, reduce hospitalizations, and improve patient
