Long range objectives 1. After 8 months of the go-live accomplishment, at least 75% of the nurses at the Minneapolis VA Medical Center will be proficient in the use and reading of I&O documentation in the electronic health record, anyone who has not been provided in-person teaching will need to complete an on-line education tutorial. (I have previously completed a hospital-wide project regarding pre-operative scrub protocol changes and to provide teaching to the entire hospital the process took around 8 months which is how I designed the long-term objective of this project).

Alford (2003) confirms, “I&O data should be collected continuously, recorded, and totaled at the end of each shift and at the end of 24 hours” (p. 229). Precise I&O documentation is indispensable to maintain patient well-being while preventing post-operative complications. To detect early signs of complications, electronic I&O charting would be able to add up I&O totals, provide flags to the provider or nurse if intake and outputs are not considered adequate. The purpose of this project planning paper is to examine the opportunity of implementing an electronic health record section dedicated to the documentation of real-time accurate I&O while terminating the inaccurate, frustrating paper I&O sheets. Expected outcomes include; elimination of double charting, decreasing delay of care and increasing collaboration between the interdisciplinary team by; having one location for the providers to access I&Os and allowing the nursing assistant to gain access to the EHR I&O section in CPRS, and having real-time documentation for when the output occurred with an accurate time frame of the nurse’s shift caring for the patient.
