On 10/03/2016 at approximately 1445 hours, at Durango Jail located at 3225 W. Gibson Lane Phoenix, AZ 85009, I was on duty as the floor officer in Durango #6. At that time I was conducting my first inmate headcount in that house. As I entered C-pod at that time the inmates inside cell 48 got my attention and said that one of their cells mates was ill. This individual was later identified as Inmate Brandon Haubrock T301090. When I entered cell 48, Inmate Haubrock was sitting on his bunk. I could see that Inmate Haubrock’s hands were shaking and that he was not responding to my questions. When I would ask Inmate Haubrock a question he would just stare at me and act like he could not hear me. His cellmates said that he had been ill lately but that this is the worst …show more content…
At approximately 1830 hours, I was advised by Sgt. Salazar that Inmate Haubrock was going to be transported to Maricopa Integrated Health System (MIHS) located at 2601 E. Roosevelt St. Phoenix AZ 85008 authorized by PA Pat Winchester # CS101 to rule out dizziness.

At approximately 1913 hours, Ambulance AMR #117 arrived at Durango Jail to transport Inmate Haubrock. Escorting Inmate Haubrock was Officer Galindo B2698 and myself. Prior to departing for MIHS, Officer Galindo informed me that at about 1842 hours, he asked Inmate Haubrock if the injury that he suffered was a result of any criminal activity that was done to him while he was incarcerated and that Inmate Haubrock responded by saying “No”. The ambulance departed Durango Jail at about 1902 hours. Our arrival at MIHS was at about 1925 hours.

At approximately 1948 hours, Officer Galindo and I were relieved at MIHS by Officer Mantooth B1838 and Officer Gill B0391. Officer Galindo and I departed for Durango Jail a few minutes later. We arrived back a Durango Jail at about 2010.
