RP took resident to Kaiser Urgent Care where they cleared 2 blood clots and emptied 2500ML of urine from his bladder. On 10/1/16, resident called RP stated that he was in pain and need to go to the ER. RP told staff that she was on his way and to have the resident dressed. When RP arrived, the resident was dress but his catheter bag was not attached. RP also reported that the resident's room was full of trash, the dirty clothes hamper was empty and a pile of clothes were wadded up on top of a plastic bin in his closet; RP did not know whether they were clean or dirty. There was no soap in the bathroom for him to wash his hands, nor paper towels to dry them. RP was told that these items were to be purchased by the family. On 10/2/16, RP stated that 2 medications were missing from the list of meds that she picked up for the resident yesterday. The meds were Flonase nose spray and Omeprazole.
