The second patient
Elsie is an 81 year old lady who lives in a sheltered accommodation. She has been self-caring all along and has had a home help for her weekly eye drops. Her daughter Sarah has been visiting her once in every week in order to make sure that she was okay. Recently she has wet to see the GP whereby he had fever, profuse sweating, and high levels of chest pains on coughing, coughs, nausea and a lot of fatigue.
From the diagnosis, Elsie was found out that she has pneumonia and he was prescribed to use antibiotics. After taking the antibiotics, she was not feeling any better because she had lost appetite and was feeling dizzy which made her to fall many times and she has been cyanosed. The RR is 24 per min and the saturation of oxygen is 93% and this has to be documented in the NEWS. I would take necessary measures of monitoring her because lower rates than 93% usually lead to hypoxemia and this shows that she has a sepsis infection (Jevron, 2010). High levels of sepsis in her body can lead to disruption of the flow of blood in the body especially the kidneys and the brain and it might lead to organic failure. This high rate is attributed to tachycardia conditions in the body (Balas, 2012). Since her heart rate is greater than 90 beats per second then she has sepsis which is caused by a variety of body insults. I would administer IV fluids because of the systolic pressure which is below 90mmHg. Elsie needs to be tested for the rate of urine output in order to determine if she has acute Liguria. If her urine output is less than 0.5ml/kg/hr., then she has hat condition.
In administering the fluids because of the hypotensive condition, she has to be given boluses of up to 0.9% and Hartmann at a level of 30ml/kg. I would also catheterize Elsie in order to be able to monitor the output of urine in an hourly level (Vaughan, 2016). However, I would be very keen because catherizing may make Elsie to be exposed to risks of infections.
Disability
I would carry out an AVPU score analysis for the patient and document it in the NEWS chart. From her condition the voice score is down which can be attributed to the respiratory problems (Parry, 2011).
