Per the medical report dated 11/21/16, the IW was first evaluated on 09/28/2016, by David Abri, general dentist, who documented that teeth #9 and 10 are painful, and that tooth #10 has a vertical fracture line. Dr. Abri referred the claimant to an endodontist (root canal specialist) to evaluate #9 and 10 for root canal treatment. It was noted that on 11/21/16, it was documented by Smar Hubbi, endodontist, upon x-rays and examination of the claimant, that teeth #9 and 10 have irreversible nerve damage. The provider also notes that tooth #11 has also sustained irreversible nerve damage. Dr. Hubbi recommends root canals for teeth #9, 10, 11. Without the recommended root canals, the claimant would likely experience increasing symptoms of pain and infection. While tooth #11 was not originally mentioned by the general dentist as being related to the injury, testing by the endodontist has determined that it has sustained nerve damage due to the trauma as well.
