Care Plans are the primary source of Patient information. Every specialty, PT/OT/Nursing etc. do an evaluation of the individual and formulate a program of goals for the patient to attain, therapies needed, schedule of goal levels, etc. Each person interacting with the patient can refer to the care plan for any information needed.
