Maybe your assessor has asked you to do an account about how you support clients at lunchtime. Look at the relevant standard so you understand what you need to be including and then simply write down what you did. Sort of like this
This lunchtime I helped clients with their food and drink. I looked in their Care Plans to find out their dietary needs and what support they needed. I helped them to get to the tables, supported them to put on clothes protectors if they wanted them and made sure they had the right cutlery, some prefer to just use spoons. I washed my hands and put on apron. And so on, just relate what you did, how you communicated with them, how and when you disposed of uneaten food, did you update any records, were there any problems, someone not eating? When you have written your account have a think and say how you think you did, were you happy with support you gave, could it have been done in a different, better way, have you had any training that helped you support clients.
A reflective account is a 'story' about a task you have completed and how you think it went.
