Every year there are several mistakes made to someone's medical chart. Almost 98,000 people die every year because someone made a mistake that could have been avoided. There are pros and cons to both the paper charts and the electronic health records. The trouble usually starts when someone requests their medical records. You have to specify which records you want which could be a generic range or a very specific range. Some patients want their entire medical record, which is fine but some Physicians charge by the page so this could be very expensive. The paper charts require additional people to be able to handle and support them. They need someone to be able to file them and be able to organize them as well. Paper files take up lots of space and it is not so safe for the environment. With so many people having paper charts and not having the needed space to store the charts many Doctor's offices has large warehouse's where they keep most of their charts. When someone requests their chart it may be time-consuming because of them not being located in the office. After locating them getting them faxed would be the fastest way to revive them. If you decide you would like them to be mailed to you that could cause a couple more days of a delay. Paper charts also are fairly easy to damage and they deteriorate over time which could cause errors when trying to read the chart. With paper charts we only have one copy, so the loss of a single record could mean that it is lost forever. A staff member could misplace it or if there is a flood or a fire there is absolutely no way to recover the files. Everyone has different handwriting so it could be hard to read what was charted by the previous staff member. There is only so much space to write your notes on so it could make the chart look sloppy if you try to squeeze everything in the space that is provided. The paper carts are often left unfinished.
