1.1 Explain what constitutes a team in an adult care setting and the different roles within the team. Firstly, in the team are the people we support and their families. Support workers, Team Leaders, Service Managers and higher management. The people we support are the centre of the support and need to be listened to and encouraged to share their views, hopes and wishes. Families bring experiences and history of the people we support and share important information and ideas with the support. Support workers who are responsible for the everyday support/care for the people in our service. Team leader to encourage, support the team, to deliver a high standard of support, to lead by example. Service manager sets the standard of support for the team to maintain. Outside agencies are also a massive part of the team, working closely with social workers, and other health professionals.

1.2 Explain why individuals, their families and carers may be considered as members of a team. The people we support are at the centre of their support/care and it is very important that they have their rights heard and respected. Individuals, families and carers become more effective in a person review or a medical appointment. The family can provide the medical history. Sharing experiences and knowledge will help with the individual support and achieving goals. Best interest meeting is important to invite the family. Medical best interest meetings are important to have the family involvement to ensure the best decision is made for the PWS. I was involved in a medical best interest case for a PWS with a lump in her breast. The most interesting meeting involved doctors, cancer nurses, management and family.
