On 8/19/16 I met Ms. Ostrander and her boyfriend at the office of Dr. Schell Neurosurgeon. Dr. Schell refuses to speak with a nurse case manager. I spoke with Ms. Ostrander before and after the appointment. Ms. Ostrander was able to walk, sit, and stand without grimacing. She does report that the chest pain is better; she said she is working on moving and not hurting herself. Her boyfriend said she is getting better whether she thinks so or not. Ms. Ostrander brought the thoracic and cervical MRI disc for doctor to review. She initially went into the exam room alone then came out for her boyfriend so he could answer questions for her. After she met with Dr. Schell Ms. Ostrander reported that he told her she has 3 disc fractures. They should heal in the next 3 to 4 months. If they do not then he would do a Kyphosis procedure. Ostrander at the office of PMR Dr. Hettle. Ms. Ostrander reports that she has no recall of the accident. That she has an indentation on the left side of her head from the accident and her left ear buzzes. She said the information she has about the accident was told to her by a witness. She told Dr. Hettle the following is areas she has pain in: the top of her head, her chest and thoracic spine, her low back, she has numbness and pain to the right and left legs and her left shoulder. Dr. Hettle examined Ms. Ostrander, I provided him with copies of radiology reports I have. The MRI of the thoracic spine was reviewed on line by Dr. Hettle. Dr. Hettle recommends doing an EMG of all four extremities, obtaining a MRI of the brain and the left shoulder. He also recommends a Neuropsychologist evaluation. He has prescribed Mobic and Flexeril for the pain Ms. Ostrander
