For this assignment I decided to interview Tara Riccitelli,she is a Pediatric Social Worker who travels to several pediatric offices throughout Rhode Island as a support staff to the Primary Care Physicians.The agency that she works for is the Rhode Island Primary Care Physicians Corporation and they are an independent practice association. The Rhode Island Primary Care Physicians Corporation is a health management organization that focuses on improving the outcome of the patients whose Physicians are members of our organization. They are an ACO (Accountable Care Organization) teaming up with Integra and Care New England that developed into a network of Primary Care Physicians, specialists and clinical services. Her original title was Social Worker but the connotation of her role entailed did not match up and it was causing some confusions between her patients. Now she follows up with her patients that are discharged from the emergency room and Inpatient department with a behavioral diagnoses to ensure that they understand the care plan they need.They are required to make an appointment with her or a behavioral health provider. She also assist patients with getting a behavioral health provider if needed. She also gets referrals from PCP that supports patients who need additional help and gets them healthcare that can provide resources that can help them. Her goal in general is to keep these patients connected to providers that they can call or go to. She also works with Children from the ages of 4-18 and adult practices.
