It is definitely possible to identify someone based on the person’s gender, birthdate, and zip code. This typically narrows down the pool of people to one or two people in the case of a doctor office’s database. There is a fairly low chance that two people will share the exacts same birthday, gender, and zip code in context of only one doctor office. Obviously this is bound to happen a couple times, however the information will still narrow down the options of patients to 3 or less people almost 100% of the time.
